Common types of drug errors

Some of the errors involve the method of administration or other clerical issues while some errors highlight specific substances most frequently involved in instances that cause patient harm or death.

Other errors happen when an initial high dose is given before dropping to a maintenance level, when prescriptions are not administered in a timely manner or are forgotten or when multiple medications negatively interact with each other.

A variety of medications have been identified as being involved in many common errors. One of these surprisingly is oxygen. When patients are given oxygen, it is critical that the correct concentration be administered. Opioids, anticoagulants and insulin have also been identified as often involved in medical errors. Nonsteroidal anti-inflammatory medications have been linked to kidney damage, heart attacks and excessive bleeding. Some drugs require regular monitoring of a patient’s blood but this monitoring may at times not occur, exposing patients to harm.
